Scientists or contractors that will be sailing overnight aboard an OMAO ship must submit a completed NOAA Form 57-10-01, Health Services Questionnaire, (NHSQ) and >NOAA Form 57-10-02, Tuberculosis Screening Document (TSD) to the Marine Medical Branch (MMB) for review no later than 30 days prior to the sailing date. Medical clearance to sail is not required for day trips. Scientists and contractors less than 18 years old must also submit a completed NOAA Form 57-10-05, Medical Form For Minors. Refer to the instructions page within the NHSQ for guidance on completing the form and a list of acceptable tuberculosis screening methods. Follow the instructions on the TSD if you have tested positive for tuberculosis exposure in the past or received a tuberculosis vaccine as a child.

Hassler, Oregon II, Thomas Jefferson, and Okeanos Explorer can be faxed to 757-441-3760 or emailed to moa.health.services@noaa.gov, or mailed to: MOA Marine Health Services 439 W. York St., Norfolk, VA 23510. After reviewing your NHSQ and TSD, someone from the MMB will contact you to request additional information or to inform you that you have been medically cleared to sail.

If you routinely sail aboard NOAA ships, you must periodically update your NHSQ and TSD to retain your medical clearance to sail. Your NHSQ remains valid for two years following the date you signed the NHSQ if you are less than 50 years old. Your NHSQ is valid for one year following the date you signed the NHSQ if you are 50 years or older. Your TSD is valid for one year after the date the screening was completed. If your TSD will expire while at sea, you must submit an updated TSD prior to sailing to retain medical clearance to sail.
