The following recommended Excel template is provided in accordance with ICR K-01 to satisfy requirements for maintaining this log. While it has not yet been approved and in the DMS, it should be helpful in the interim and we encourage you to use it. In addition to preformatted log sheets for the next few years, a tab for references is provided with all of the latest updated requirements found in the CFRs, International Maritime Organization, and OMAO procedures. Guidance for formatting and data entry is provided by clicking the first tab at the bottom left called "Guidance".
