Deadline for Secondary Application Materials (NOAA Diver May Class)

Government employees wishing to take the NOAA Diver Classes offered May 8-26 (modules 1, 2, and/or 3) are required to submit the final portion of their application materials:

• NF 57-03-50, 51, 52, Report of Physical Examination – Diver
• NF 57-03-39, Swim Test Evaluation
• NF 57-03-41, Diving Activity Resume
• NF 57-03-09, Liability Release and Assumption of Risk

The initial applications should have been submitted by March 17, 2017.

Medical forms should be sent via Accellion or fax at 206-529-2759. All other materials may be sent to support.ndc@noaa.gov.
