Use this form to request the acquisition of a vessel to be used by NOAA regulated under the NOAA Standards and Procedures Manual. Vessel may be new construction or an existing vessel from another agency. Mouse over each block for a further detailed explanation of the information requested. This form may only be originated by Vessel Operations Coordinators (VOC) or their designee. This form shall be routed through your Vessel Program Coordinator (VPC) if any, and to your respective Line Office Small Boat Officer (LOSBO).
