Scientists and contractors who will be sailing overnight aboard an Office of Marine and Aviation Operations (OMAO) ship must submit a completed NOAA Form 57-10-01, NOAA Health Services Questionnaire (NHSQ) and NOAA Form 57-10-02, Annual Tuberculosis Screening Document to the Office of Health Services, Marine Medicine Branch (MMB) for review no later than 30 days before the sailing date. Medical clearance to sail is not required for day trips. Scientists and contractors less than 18 years old must also submit a completed NOAA Form 57-10-05, Medical Form For Minors

If you routinely sail aboard NOAA ships, you must periodically update your NHSQ and Tuberculosis Screening Document to retain your medical clearance to sail. Your NHSQ remains valid for two years following the date you signed the NHSQ if you are less than 50 years old. Your NHSQ is valid for one year following the date you signed the NHSQ if you are 50 years or older. Your Tuberculosis Screening Document is valid for one year after the date the screening was completed. If your Tuberculosis Screening Document will expire while at sea, you must submit an updated NOAA Form 57-10-02 before sailing to retain medical clearance to sail.
